When You Should Use This Form
You should only use this form if you are not able to attend the AGM in person but you wish to join us via Zoom.
If you wish to attend the AGM in person, you can submit your booking for the AGM (you can also book for the Lunch) using this form: Booking Form for the Annual Lunch and AGM
If you are not able to attend the AGM in person, you can submit a request for remote participation via Zoom using the form below.
If you are not able to join us via Zoom, please submit your Apology/Proxy using this form: Apologies/Appointment of Proxy
If you wish to participate in the AGM via Zoom, you do not need to submit an Apology/Proxy.
