When You Should Use This Form

You should only use this form if you are not able to attend the AGM in person but you wish to join us via Zoom.

• If you wish to attend the AGM in person, you can submit your RSVP for the AGM using this form: RSVP for 2025 AGM
• If you are not able to attend the AGM in person, you can submit a request for remote participation via Zoom using the form below.
• If you are not able to join us via Zoom, you can submit a Proxy using this form: Appointment of Proxy
• If you wish to participate in the AGM via Zoom, you do not need to submit an Apology or a Proxy.
